Communicare is dedicated to enriching the quality of life for persons with acquired brain injury, neurological impairment and individuals with developmental disabilities by providing community living opportunities combined with meaningful, productive and therapeutic daily activities.
The goal of Communicare is to provide each resident with the opportunity to live in a community environment and access community vocational, educational and recreational opportunities. Communicare provides support, supervision and guidance to ensure that each resident has the opportunity to experience the quality of life to which we all aspire: security, identity, community involvement, as well as meaningful vocational activities, recreational pursuits and social support. The residents of Communicare reside in community based homes in the greater Tampa Bay area. This shared living arrangement provides opportunities for immediate and meaningful social relationships. The residents are encouraged to be responsible for managing the household budget, shopping, cooking, completing laundry tasks and performing general household maintenance. Performing these activities of daily living provide a greater sense of independence and identity for each resident. Support and supervision are provided 24 hours a day, 8 days a week, 365 days a year.
Coordinated daily activity programs ensure that each resident is provided avocational, vocational and recreational activities. Vocational activities include competitive, supported, and volunteer employment. Job seeking skills training, social skills acquisition, and staff support are provided to ensure each resident sees himself or herself as a participant and not a patient. Recreational and leisure activities are an important component of the program. In addition to making life fun, the recreational activities broaden ones identity and enhance the development of social relationships. For those residents interested in pursuing educational options, encouragement and daily assistance will be provided to access the excellent academic options available in the community.
A work hardening program provides residents resources at home as well as within the local community to participate in vocational opportunities. Based upon ability, options, and productivity, residents may be employed and receive wages for services in areas such as: Horticultural, Woodworking Activities, Automotive Detailing, Landscaping, Maintenance and Clerical/Computers.
Brain Injury Residential Rehabilitation/Residential Habilitation
The program is community based and addresses the unique medical, physical, cognitive, psychosocial, behavioral, vocational, educational and recreational needs of the persons served. The Residential program provides specific training activities that assist the persons served to acquire, maintain or improve skills related to activities of daily living. The service focuses on personal hygiene skills, homemaking skills and on social and adaptive skills that enable the persons served to reside in the community. A more intensive Residential service may be required for persons served with challenging behavioral disorders. The necessity for intensive service is determined by the impact of these behavioral disorders on the immediate safety, health, progress and quality of life for the persons served and the individuals within the persons’ served environment. This program encompasses care that enhances the lives of the persons served within their families/support systems, communities, and life roles. This service, at minimum, shall include:
- Access to information about services available within the coordinated continuum of care.
- Movement through the continuum of care
- Conservation of funding to meet life-long needs
- Linkages with the community
- Family / support system support
- Education of the persons served, their families / support systems, and the community
- Facilitation of opportunities for interactions with individuals with like activity limitations
- Focus on reducing challenging behaviors and in acquiring new skills (replacement behaviors)
Home- and Community- Based Rehabilitation / Outpatient Services
The service provides integrated, case-managed, outcomes-focused rehabilitation, training and assistance in a variety of activities, to support the persons served who live in their own homes or apartments. This service has a primary focus of, but is not limited to, the maintenance, generalization, or improvement of skills related to activities of daily living and the social, adaptive, vocational and recreational / leisure skills necessary to enable the persons served to reside on their own. To aid the persons served to successfully achieve their outcome goals, the services provided may include on-site staff supervision to ensure the health and safety of persons served, transportation, vocational training, etc…
Sustained Care- Long Term Residential Services
Person who has suffered a traumatic brain injury or other neurologic impairment, who, based on neuro-cognitve and neuro-behavioral intervention, require, at a minimum, a community based environment with twenty-four hour supervision for quality of life activities, i.e., activities of daily living, budgeting, laundry, assistance with medication, medical follow-up and community inclusion activities.
